      Where should my GP referral and mental health treatment plan get sent?

      You can request that your GP send the documents directly to us, or you can ask for a copy and send them yourself.

      Ask your GP to send your MHCP to Someone.health.

      Fax: 02 8367 5101

      It doesn’t matter who the letter is addressed to – it can either be the name of your psychologist or just address to Someone.health. 

      If you’re using a Someone.health GP, we’ll already have your referral so you won’t need to send it anywhere.
