When you're talking with your GP, you need to request a mental health care plan and referral for your Someone.health psychologist.
They'll know exactly what you're talking about and will have the paperwork readily accessible.

If you've already booked a psychology appointment and have chosen a psychologist, you can ask your GP to address the referral and mental health treatment plan directly to them.
Otherwise, it can just be addressed to:
