If you've had a mental health treatment plan in the last 12 months, you're not eligible for a new one under Medicare.
If you've had a plan in the last year, reach out to the the GP Practice you originally went to and ask for a copy, or request they send your plan to us via email referrals@someone.health or fax 02 8367 5101.
