Where should my GP referral and mental health treatment plan get sent?

You can request that your GP send the documents directly to us, or you can ask for a copy and send them yourself.

Ask your GP to send your MHCP to Someone.health.

It doesn’t matter who the letter is addressed to – it can either be the name of your psychologist or just address to Someone.health.
